Nucleoside analogue reverse transcriptase inhibitor options: a re-examination of the class.
نویسنده
چکیده
The main options for dual nucleoside (or nucleotide) analogue reverse transcriptase inhibitors (nRTIs) as a component of initial antiretroviral therapy regimens are tenofovir/emtricitabine, zidovudine/lamivudine, and abacavir/lamivudine as fixed-dose combinations. Resistance to nRTIs can limit usefulness of many of the drugs in the class. Investigation of triple nRTI regimens has shown that zidovudine/lamivudine/abacavir does not provide benefits compared with dual nRTIs plus efavirenz and that others (tenofovir/lamivudine/abacavir and didanosine/lamivudine/abacavir) are associated with very high virologic failure rates. Further, 4-nRTI regimens are under investigation. The article summarizes a presentation on nRTIs made by Scott M. Hammer, MD, at the International AIDS Society-USA course in New York in March 2006. The original presentation is available as a Webcast at www.iasusa.org.

Mitochondrial toxicity of nucleoside analogue reverse transcriptase inhibitors: a looming obstacle for long-term antiretroviral therapy?
Nucleoside reverse transcriptase inhibitors suppress HIV replication by blocking reverse transcriptase, an RNA-dependent DNA polymerase. These drugs can also affect cellular and mitochondrial DNA polymerases. Mitochondrial DNA polymerase gamma is particularly sensitive to nucleoside reverse transcriptase inhibitors, and the majority of adverse effects caused by nucleoside reverse transcriptase ...
متن کاملDrug- Resistance- Associated Mutations and HIV Sub-Type Determination in Drug-Naïve and HIV-Positive Patients under Treatment with Antiretroviral Drugs
Abstract Background and Objective: Resistance to antiretroviral agents is a significant concern in clinical management of HIV-infected individuals. Resistance is the result of mutations that develops in the viral protein targeted by antiretroviral agents. Material and Methods: In this cross-sectional study, the blood samples of 40 HIV-positive patients were collected. Twenty of them were d...
متن کاملNucleoside analogue-sparing strategy for the treatment of chronic HIV infection: potential interest and clinical experience.
Nucleoside analogue-sparing antiretroviral combinations may be interesting as first-line therapies as they spare a complete class of drugs that will remain fully active for later use and prevent the risk of mitochondrial toxicity related to exposure to nucleoside reverse transcriptase inhibitors (NRTIs). This strategy is also used in patients failing NRTIs with cross-resistance to compounds in ...
متن کاملSequential transmission and long-term persistence of an HIV strain partially resistant to protease inhibitors.
Transmission of drug-resistant HIV-1 variants has been increasingly documented. The most commonly observed resistance-associated mutations are thymidine analogue mutations as well as non-nucleoside reverse transcriptase inhibitor mutations. We report on a case of secondary transmission of a protease inhibitor (PI) primary mutation from an infected untreated subject to his sexual partner. Sequen...
متن کاملSafety and Toxicity of Individual Antiretroviral Agents in Pregnancy NUCLEOSIDE AND NUCLEOTIDE ANALOGUE REVERSE TRANSCRIPTASE INHIBITORS
There are currently six approved nucleoside analogue reverse transcriptase inhibitors. Data are available from clinical trials in human pregnancy for zidovudine and lamivudine, while didanosine and stavudine are under study. Zalcitabine and abacavir have not been studied in pregnant women. Tenofovir disoproxil fumarate is the first acyclic nucleotide analogue reverse transcriptase inhibitor. Th...
